We’re finishing our review of the 2022 ICD-10-CM Official Guidelines for Coding and Reporting changes in C. Chapter-Specific Coding Guidelines.

Chapter 15: Pregnancy, Childbirth, and the Puerperium (O00-O9A)

a. General Rules for Obstetric Cases

3) Final character for trimester

Before 2022, guidance was provided that when there was an “in childbirth” option for the obstetric complication when delivery occurs, the “in childbirth” code should be assigned.

Effective for 2022, when the ICD-10-CM classification does not provide an obstetric code that includes an “in childbirth” option, we assign the ICD-10-CM code that describes the current trimester.

a.  Alcohol, tobacco, and drugs use during pregnancy, childbirth, and the puerperium

1) Alcohol use during pregnancy, childbirth, and the puerperium

3) Drug use during pregnancy, childbirth, and the puerperium

The only change for 2022 is the word “mother” has been changed to “patient.”

Chapter 18: Symptoms, signs, and abnormal clinical and laboratory findings, not elsewhere classified (R00-R99)

ICD-10-CM code R40.20Unspecified coma may be assigned in conjunction with codes for any medical condition.

Another important note: “If multiple coma scores are captured within the first 24 hours after hospital admission, assign only the code for the score at the time of admission. ICD-10-CM does not classify coma scores that are reported after admission but less than 24 hours later.”

Chapter 19: Injury, poisoning, and certain other consequences of external causes (S00-T88)

d.  Coding of Burns and Corrosions

6) Burns and corrosions classified according to extent of body surface involved

Codes from category T31 or T32 should be assigned “for acute burns or corrosions when the site of the burn or corrosion is not specified or when there is a need for additional data.

Codes from categories T31 and T32 should not be reported for sequelae of burns or corrosions.”

Chapter 21: Factors influencing health status and contact with health services (Z00-Z99)

b.  Z Codes Indicate a Reason for an Encounter or Provide Additional Information about a Patient Encounter

4) History (of)

This clarification caused me to do a happy dance!  The reason for the encounter (for example, screening or counseling) should be sequenced first and the appropriate personal and/or family history code(s) should be assigned as additional diagnos(es).

10) Counseling

New ICD-10-CM code Z71.85 – “Encounter for immunization safety counseling, is to be used for counseling of the patient or caregiver regarding the safety of a vaccine. This code should not be used for the provision of general information regarding risks and potential side effects during routine encounters for the administration of vaccines.

17) Social Determinants of Health (New section)

coding professionals may utilize documentation of social information from social workers, community health workers, case managers, or nurses, if their documentation is included in the official medical record.”

Chapter 22: Codes for Special Purposes (U00-U85)

For the new ICD-10-CM code U09.9, the guidelines instruct us to see Section I.C.1.g.1.m.

Chapter 3: Disease of the blood and blood-forming organs and certain disorders involving the immune mechanism (D50-D89)

No updates

Chapter 4: Endocrine, Nutritional, and Metabolic Diseases (E00-E89)

3) Diabetes mellitus and the use of insulin, oral hypoglycemics, and injectable non-insulin drugs

Before 2022, ICD-10-CM instructed us to code only Z79.4 – Long-term (current) use of insulin when patients were treated with both oral medications and insulin. However, 2022 guidelines instruct us that if patients are treated with both oral medications and insulin, Z79.4 – Long term (current) use of insulin, and code Z79.84 – Long term (current) use of oral hypoglycemic drugs should be assigned.

6) Secondary diabetes mellitus

(a) Secondary diabetes mellitus and the use of insulin, oral hypoglycemic drugs, or injectable non-insulin drugs

Same change as above.  Before 2022, ICD-10-CM instructed us to code only Z79.4 – Long-term (current) use of insulin when patients were treated with both oral medications and insulin. However, 2022 guidelines instruct us that if patients are treated with both oral medications and insulin, Z79.4 – Long term (current) use of insulin, and code Z79.84 – Long term (current) use of oral hypoglycemic drugs should be assigned

Chapter 5: Mental, Behavioral and Neurodevelopmental disorders (F01 – F99)

4) Medical Conditions Due to Psychoactive Substance Use, Abuse and Dependence

This is new guidance.  “Medical conditions due to substance use, abuse, and dependence are not classified as substance-induced disorders. Assign the diagnosis code for the medical condition as directed by the Alphabetical Index along with the appropriate psychoactive substance use, abuse or dependence code.”

5) Blood Alcohol Level

Also new guidance.  “A code from category Y90, Evidence of alcohol involvement determined by blood alcohol level, may be assigned when this information is documented and the patient’s provider has documented a condition classifiable to category F10, Alcohol related disorders. The blood alcohol level does not need to be documented by the patient’s provider in order for it to be coded.”

Chapter 6: Diseases of the Nervous System (G00-G99)

No updates

Chapter 7: Diseases of the Eye and Adnexa (H00-H59)

No updates

Chapter 8: Diseases of the Ear and Mastoid Process (H60-H95)

No updates

Chapter 9: Diseases of the Circulatory System (I00-I99)

No updates

Chapter 10: Diseases of the Respiratory System (J00-J99), U07.0

We’re reminded to “See Section I.C.1.g.1.c.i. for Pneumonia confirmed as due to COVID-19.”

Chapter 11: Diseases of the Digestive System (K00-K95)

No updates

Chapter 12: Diseases of the Skin and Subcutaneous Tissue (L00-L99)

2) Unstageable pressure ulcers

We know that when there’s no documentation of the ulcer stage to assign code for unspecified stage. However, according to the 2022 guidelines, if the stage is known after debridement, assign just the code for the known stage post-debridement.

Chapter 13: Diseases of the Musculoskeletal System and Connective Tissue (M00-M99)

e. Multisystem Inflammatory Syndrome

This is a new section, and 2022 ICD-10-CM guidelines instruct us to “See Section I.C.1.g.1.l for Multisystem Inflammatory Syndrome.”

Chapter 14: Diseases of Genitourinary System (N00-N99)

No updates

Let’s start reviewing the 2022 ICD-10-CM Official Guidelines for Coding and Reporting changes in C. Chapter-Specific Coding Guidelines.

Chapter 1: Certain Infectious and Parasitic Diseases (A00-B99), U07.1, U09.9

a. Human Immunodeficiency Virus (HIV) Infections

            (d) Asymptomatic human immunodeficiency virus

“HIV disease” should not be coded as Z21, Asymptomatic human immunodeficiency virus [HIV] infection status.  The term “HIV disease” has been added to the instructions to report ICD-10-CM code B20. 

  • History of HIV managed by medication (new guideline)

Report code B20, Human immunodeficiency virus [HIV] disease when providers document, “History of HIV.”  Z79.899, Other long-term (current) drug therapy, may be assigned as an additional code when documentation supports the long-term (current) use of antiretroviral medications.

g. Coronavirus infections

(g) Signs and symptoms without definitive diagnosis of COVID19

The diagnosis code for Cough (R05) changed to new codes R05.1, Acute cough, or R05.9, Cough, unspecified.

(j) Follow-up visits after COVID-19 infection has resolved

Z09, Encounter for follow-up examination after completed treatment for conditions other than malignant neoplasm should only be reported when providers document that patients do not have residual symptom(s) or condition(s) associated with COVID-19.

“For follow-up visits for individuals with symptom(s) or condition(s) related to a previous COVID-19 infection, see guideline I.C.1.g.1.m.”

(l) Multisystem Inflammatory Syndrome

When providers document that patient with a history of COVID-19 develops Multisystem inflammatory syndrome, assign code U09.9 -Post-COVID-19 condition unspecified as an additional diagnosis.  The primary diagnosis is M35.81.

(m)Post COVID-19 Condition (new guideline)

Remember we reviewed manifestation (condition caused by (due to) the underlying disease)  and sequelae (residual/late effects)?  Well, here we go…

“For sequela of COVID-19, or associated symptoms or conditions that develop following a previous COVID-19 infection, assign a code(s) for the specific symptom(s) or condition(s) related to the previous COVID-19 infection, if known, and code U09.9, Post COVID-19 condition, unspecified.

Code U09.9 should not be assigned for manifestations of an active (current) COVID-19 infection. If a patient has a condition(s) associated with a previous COVID-19 infection and develops a new active (current) COVID-19 infection, code U09.9 may be assigned in conjunction with code U07.1, COVID-19, to identify that the patient also has a condition(s) associated with a previous COVID-19 infection.

Code(s) for the specific condition(s) associated with the previous COVID-19 infection and code(s) for manifestation(s) of the new active (current) COVID-19 infection should also be assigned.”

2. Chapter 2: Neoplasms (C00-D49)

s. Breast Implant Associated Anaplastic Large Cell Lymphoma

Do not assign ICD-10-CM code for a complication when providers document breast implant associated anaplastic large cell lymphoma.   Assign code C84.7A, Anaplastic large cell lymphoma, ALK-negative, breast, for BIAALCL.
